Disenrollment Form

We understand that sometimes there are unforeseen circumstances that cause a child to have to disenroll from their extracurricular activities (ie: move, death in the family, illness or injury).

There are also situations when a student does not want to continue with their activity and they want to switch and try something new. We completely understand that.

HOWEVER, as in most situations in life, we must fulfill our obligations and commitments to complete (to the best of our ability barring the unforeseen situations previously mentioned) our instructions as scheduled by the CYM Enrichment Partner.

If you decide to discontinue instructions/classes/seasons/sessions/ please first inform the Enrichment Partner of your desire to discontinue and disenroll. Please note who you spoke with, the date and time.

You will be expected to put that information in the form below. When the form is totally complete, hit the SUBMIT button. This Disenrollment Form will then be sent to both the Director of Admissions and to the CYM Bookkeeper informing them of your intentions.

We are certain you will agree, that the financial support that your child has received for their extracurricular activity, has been very helpful (& very costly). We want to be responsible to our donors and not pay for services that your child isn’t receiving. We also want to be respectful to our Enrichment Partners and not waste their time and effort, if you student does not attend.

We must have this form completed before being removed from classes. Thank you for your time and attention to completing this process.

In order to discontinue any class, at least 30 days notice from the first of the month must be given.
